Log out, clear your browser cache and cookies, restart your browser and then take another look at the contact form before logging in and you will see the fields are blank.

As wpgaurav has pointed out we need you to post an active link starting with http:// to the blog in question please. If you are referring to a free hosted WordPress.com blog then when you set your blog to private, you can add up to 35 users to your access list from Settings -> Privacy free of all charges.

If you are running a larger blog community, then you will need to purchase the Unlimited Private Users upgrade in order to continue adding fellow WordPress.com users. The upgrade is per blog and is available for purchase in the Store tab of your dashboard.

None of your official users have to pay $12.00 they can scroll down that form and choose "Just a username, please ".
