You can switch to three columns in the Elegant Grunge theme using the "Content-Sidebar-Sidebar" layout option in the Appearance → Theme Options page in your blog dashboard.

The Elegant Grunge theme is designed using images in the background to create the column effect. In order to create a new column in a theme designed like that, the first thing you would need to do is create a new image to use for the background.
