Create the page, go to the discussion module in the page editor and turn off comments and trackbacks (if it is not visible, click screen options at upper right of the page and activate it), then switch to the HTML tab in the page editor and add the contact form there.

So, I've been trying to add a custom form to my webpage so people can contact me for bookings etc, and whenever I do, nothing appears at all, anywhere!
I've tried making a new page to see if this fixes the problem, but so far, no luck - any ideas?!
