Can someone point me in the right direction, I am looking to get the script for such a form. I am an author and would like to set up a JV page w/bonuses when I launch my book and I would like the people who have purchased the book to have access after they have input their name, email address and VALID amazon receipt.

Any help or ideas or person that can do this would be appreciated.
