Make a dummy post with the desired image and set your Custom URL (to http://example.wordpress.com/?random) under Attachment Display Settings when inserting image. Copy and paste code from said dummy post into a text widget.

I have seen this on other blogs and I think it is a great addition. I usually see it at the end of a post or in a footer...
