All upgrades are per blog, per year. If you transfer the content to a different host, the upgrades do not come along.

Note that at most independent hosts they are not necessary, as you have many of those things like Custom Design automatically.

I have a new domain name with them as well. I still have 6 months left on my custom design account that I want to transfer to the new site with the new domain name http://www.accessyourawareness.com - how do I do this ?? the blog that I originally purchased this upgrade with is intuitiveawareness.ca - which is probably now intuitiveawareness.wordpress.com as the domain has expired.
