Every time I try and add a contact form the code comes up and then when I go to publish, it totally vanishes, I've tried it a number of times, can anyone tell me what I'm doing wrong or is something else at fault?

All you do is edit that page and type in the contact form shortcode [contact-form] and publish the page. If you wish you can chose another shortcodcode that suits your needs > http://en.support.wordpress.com/contact-form/#shortcode-reference
