when someone writes me a comments and I answer them back, Do they get to see that on their email adress? if not How do i get to do that ? can they send the author a question and i get it on my email ?

If they subscribe to the comments they will see it. If they don't, they won't.
If you use a Contact Form, you can get questions via email that don't show up in the comments.

One more question as you seems to be pretty good at this.
If you go my site, what would you do to improve it.. I am quiet new at this and I am not sure I like my site. I just wrote a book about The Joy of Cancer , doing very well, but want to do more ...any suggestions would be really appreciated. Do you think i have the right theme for what I want to do ?

How can I put something for someone to subscribe to my blog?
I'm afraid I don't have time to do a blog analysis, but I can tell you that the Email Subscription widget is in your Widget list and is essential. Be sure to add in appropriate services in your Settings->Sharing page too.
