I would like to move my blog to the Fresh & Clean theme, but I hate the cursive, Disney-esque font of the title.
Do I need a custom design upgrade to do this?
Also, with the Fresh & Clean theme, could I have a picture header at the top of each post - like with splendio? Would getting a custom upgrade allow me to do that?

You will need the custom design upgrade to change title fonts, yes.
You might be able to get images on the Fresh and Clean theme, which is specifically designed NOT to show images on the main page, by manually pasting all the code for the image in the Manual Excerpt box beneath the Write Post box of the New Post page.
