I'd like to change the name and the address of the "linked blog" I have on the internet:
Name: ciltyhepooh.it => blog 42mm
Address: http://www.cilythepooh.it => blog.42mm.it
My account is: 42mm.wordpress.com
How can I do it?
(If I got it right, the only way is asking the support team to do it for me....)

and that is a wordpress.com issue.

It can't actually be changed.
I can remove it if you'd like, then you can re-link it with the Jetpack plugin.

It looks like the fix only worked for a few folks. We're still looking into this.
It is very strange as it disappeared (i checked just before I wrote my TNX message, Oct 1, 2011, 10:29 PM) but the day after was back again...

Are you trying to use Jetpack or the WordPress.com Stats plugin?
I have removed it again, but you need to use Jetpack.
I installed jetpak on the blog http://www.cilythepooh.it and connected to an account on wordpress,con, cilythepooh.wordpress.com other than mine.
Then I installed jetpack on http://blog.42mm.it and connected to my account 42mm.wordpress.com.
I'll see what is going to happen.
