I've followed the written and video explanation and it seems very straight forward but the contact form will not appear. any ideas?

Are you using Internet Explorer? It doesn't quite work properly with contact forms. Try Google Chrome.

I see the form just fine. Have you resolved the issue or is it still invisible to you?
