I put in a contact form in my "About Me" page using the built-in little button next to the polls button and the Add Media button. When I set it up, the form was blank, but when it went live on my page, it had MY email and website and name in the fields instead of being blank for my readers to fill out. How can I fix this!? Thanks so much!!!

Nothing to fix. If I were to visit your form, my data would be pre-filled.
Only you see your own data.
And, you only see it when you are logged into WordPress.com. If you log out and clear your cache and cookies and visit your blog without logging in, then you won't see it either.
