In your Dashboard go to Options >>> Reading and then scroll down to "Before a comment appears." Fill out that section as you see fit.
if one chooses that option for admin purposes. How does it work? How can someone get a login (log/pass) if they want to comment and never have before?
Well, I don't think you can necessarily password-protect your comments. But anyone commenting will have to provide you with a name and e-mail address.

Actually they wouldn't have to if you unchecked Dashboard -> Options -> Discussion -> Comment author must fill out name and e-mail. There's another thread here somewhere that mentions the boxes are still there though. And of course they can throw in whatever they want if you think about it. I usually use [email redacted] myself...
Now, another thig you could do is Dashboard -> Options -> General -> "Users must be registered and logged in to comment" and that would require them to have both a WP.com account and be logged in to comment. That might be what you're thinking of. Of course that'll limit the number of comments you'll get...
