For blogs hosted here at WordPress.com, we do not have the option to add in our own widgets and forms are stripped due to security concerns.
If your blog is hosted elsewhere (and I'm guessing that since you've been here for a total of 4 minutes) you need to be over at http://wordpress.org/support as you would be running different software than we are here.
