I used to have a blog on wordpress.com called “lmunck.wordpress.com”, but now it has disappeared. When I try to create it again it says the site already exists, and when I go to the URL it says it is no longer available.
What am I doing wrong?

It’s unfortunate that you deleted the blog by using these instructions that include 3 warnings and require clicking a verification link in an email. The warnings and verification email state that deleted blog URLs are not recycled. http://en.support.wordpress.com/delete-site/
You can register another blog with a similar name. http://en.support.wordpress.com/register-a-blog/
Or you can register a domain via a domain mapping upgrade and it will not matter what the underlying .wordpress.com address is.
