Transfer external blog to another user

  I have a blog in my list, but this blog is not mine. I made this blog on my previous work. Now I need transfer this blog to the real owner. How I do it?

  My account user is: marcmascort
  Blog: http://www.teresabaro.com (This blog is outer than wordpress.com but appear in my blog list)
