I apologize in advance if this question has already been answered – my searches didn’t really lead anywhere.
What I want to do is allow the user to upload an image for their entry that will act as part of the entry title (as on http://www.blind.com)
If the user just uploads an image, it will be inline with the rest of the entry, which is not what I want. I want it to be a headline image that spans the width of the Title DIV. If they just upload an image it will fall in the padding of the content DIV. At least…that’s how I’m setting up my CSS document.
I’m assuming I’ll need a plugin to accomplish this?

There is no way here at wordpress.COM to have images in post or page titles. You could probably do something like that with a self-hosted install of wordpress, but it would take some experience with PHP scripting to modify the underlying theme files and some expertise with CSS.
And actually, the blog linked to your username is not a wordpress.COM blog, but a self-hosted wordpress blog. You need to head over to http://wordpress.ORG/support/ as that is where self-hosted wordpress blogs are supported. The software here at .COM is quite different and we have a lot more restrictions.
You could have an image as a part of the post or page title here at WP.com but it would be the SAME image for every post or page (using CSS, of course). I’ve played with it on my blog but never been completely happy with the result.
